Us
Health Roads is a boutique consulting firm based in Newark, California. The company was founded in January of 2018 by Rajib Ghosh, MS, MBA, with years of prior experience in implementing Health and Human Services IT systems in local government and community-based organizations. Mr. Ghosh currently serves as the CEO of company.
To date Health Roads has worked for health care services organizations of counties and local community-based health and human services organizations in twenty-seven states around the country.

California’s Department of Health Care Services has introduced Providing Access and Transforming Health (PATH) initiative to support California Advancing and Innovating Medi-Cal (CalAIM) program. It is a 5-year, $1.85 billion initiative to build up the capacity and infrastructure of community-based organizations (CBOs), public hospitals, county agencies, Medi-Cal Tribal and designees of Indian Health Programs, and others, to successfully participate in the Medi-Cal delivery system as California implements a multi-year program of Enhanced Care Management (ECM) and Community Supports and Justice Involved initiative under CalAIM.

The purpose of the PATH funding is to assist local organizations to build organizational capacity and infrastructure to scale up their services they provide to Medi-Cal beneficiaries. The inspiration and learnings behind this came from the Whole Person Care and Health Home Pilots that were conducted by the State for several years before CalAIM initiative was phased in. Health Roads has deep experience in supporting all of those programs including transitions to CalAIM from Whole Person Care.

The resources funded by PATH—such as additional staff, billing systems, and data exchange capabilities— are intended to support community partners to successfully contract with managed care organizations, bringing their wealth of expertise in community needs to the Medi-Cal delivery system.

Health Roads in conjunction with its collaborators and partners have successfully assisted counties and under-resourced CBOs to expand their revenue opportunities using CalAIM while addressing social deterrents of health in a more equitable and accessible manner.

Public Health EHR Consulting
As part of this service offering, we work with our public health clients to understand their EHR requirements, help them to analyze and optimize their existing workflows and map their workflows to their existing or newly procured system. We also work with our clients to develop Request for Proposal to procure EHR or Client Case Management systems and help in system implementation. We also partner with EHR vendors to implement their IT system within their client environments. Some of our clients include Alameda County Department of Health, Baltimore County Department of Public Health, El Dorado County Department of Public Health, and Oklahoma State Department of Health.

Human Services IT Consulting
Impactful information technology services to support management of homelessness within a county region is a key offering from the Health Roads team. Our team brings many years of experience developing and implementing a range of homeless management information systems (HMIS) based solutions in multiple states. Community members served through our work included all HUD target populations and subpopulations (homeless individuals, homeless households with adults and children, homeless transitional aged youth, homeless youth 18 and under, persons who are pregnant and parenting, persons at risk of homelessness, survivors newly homeless due to wildfire and floods, veterans, persons with serious mental illness and/or substance use disorders and those impacted by racial disparities). We have helped organizations in fourteen states that are focused on HIV program, youth education, LGBTQ health programs, worker training programs, senior care, immigration and refugee assistance, and many other social care services with their implementation of Human Services IT systems.

Social Health Information Exchange (SHIE) OR Community Information Exchange (CIE) Consulting
We have deep and unique expertise in combining medical and human services data into coherent longitudinal records for clients. Health Roads is an integral part of a SHIE system developed by Alameda County of California to deliver whole person care to the community’s vulnerable constituents. We will be starting another similar engagement in Sacramento County shortly. We are working with a CIE in Washington State to implement a Health IT system for a multi-sectoral referral network.
