On April 28th, we introduced NextCareEngage™, the newest addition to the Health Roads platform, in a setting that reflected exactly what this product stands for—human connection, creativity, and purpose. Hosted at the Art of SF Gallery, the evening wasn’t just a product launch. It was a statement about how meaningful innovation actually happens.
The gallery’s mission, supporting DrawBridge Arts and its work with under-resourced children, mirrored our own belief: systems that serve vulnerable populations must be built with empathy, not distance. When art and healthcare meet, something deeper than technology emerges - intent.
The Philosophy Behind NextCareEngage™
am a firm believer that the best ideas don’t come from isolation. They emerge from proximity to real problems.
NextCareEngage was not built in a lab. It was shaped in the field. Alongside care coordinators, ECM providers, community health workers, and organizations, we navigate the complexity of Medicaid populations every day. One of our ECM provider customers received 8,000 ECM-eligible members from one of their Managed Care Health Plans, but was able to reach out to fewer than 50 members and enroll fewer than 10 using their phone systems and 2 care coordinators. In the end, what was on the paper and what actually transpired left a gaping hole between those two realities. The provider eventually had to furlough their care coordinators because revenue from the CalAIM contract did not meet expectations, which prevented the outreach workers from being kept on! We found them at this juncture and tried to develop a solution to address such acute crises. NextCareEngage was born from such burning decks.
This product reflects years of listening:
Listening to providers struggling to reach members who don’t answer calls
Listening to care teams balancing overwhelming caseloads
Listening to systems that fail not because of a lack of effort, but a lack of coordination
NextCareEngage is our answer to those realities.
What NextCareEngage Does
At its core, NextCareEngage is an Agentic AI and Machine Learning platform designed to power:
Intelligent outreach prioritization
Personalized engagement across voice, SMS, and email
Enrollment acceleration into ECM and Community Supports
Continuous learning from real-world interactions
But what makes it different isn’t just the technology.
It’s the intent behind every interaction.
The platform uses a patent-pending algorithm that prioritizes outreach based on signals that matter—timing, vulnerability, behavioral patterns, and care context. Every recommendation is grounded in domain expertise built over decades.
This is not AI built to impress investors. This is an AI built to help providers win the next payer contract and ensure the right person gets care at the right time.
From Outreach to Outcome: Completing the Trifecta
NextCareEngage is not a standalone tool. It completes what we call the Health Roads Trifecta:
QConnect → Connects fragmented systems and enables compliant data exchange
NextCareEngage → Activates outreach, engagement, and enrollment
SocialRCM → Converts services into accurate, timely reimbursement
Together, they form a closed-loop system:
Find → Connect → Engage → Enroll → Bill → Sustain
This is what modern Medicaid infrastructure requires—not point solutions, but coordinated ecosystems.
A Conversation That Grounded the Vision
The evening also featured a candid panel discussion with customers and community leaders who work on the front lines of care delivery.
The conversation was honest and necessary:
Workforce shortages and burnout
The difficulty of reaching high-need populations
Fragmented systems that slow down care
The pressure to demonstrate outcomes in value-based models
What emerged was clear: technology must reduce friction, not add to it.
NextCareEngage is designed to operate as a force multiplier—extending the reach of care teams without replacing the human touch that defines this work.
Why This Launch Mattered
Launching at an art gallery, in partnership with an organization like DrawBridge Arts, was intentional. Healthcare, especially for vulnerable populations, is not just operational; it is human.
Just like art, it requires:
Interpretation
Empathy
Timing
Presence
NextCareEngage reflects that philosophy. It brings structure to chaos, intelligence to outreach, and continuity to care journeys that are often fragmented.
What Comes Next
We are entering a phase where Medicaid programs, community-based organizations, and managed care plans must do more with less—while delivering better outcomes.
That requires:
Smarter prioritization
Faster engagement
Measurable impact
NextCareEngage is built for this moment.
And like everything we build at Health Roads, it will continue to evolve. But, not in isolation, but alongside the people doing the work every day.
Because the future of care isn’t built in towers. It’s built in the field.
